A worker was completing a task on the grid mesh floor above several tanks. (Approx 30mtrs) Because entry was needed to work on the tank below, a 2mtr section of grid mesh had been removed and the appropriate
Hard Bar erected around the floor opening. Caution Tape or Do Not Enter was not wrapped on the Hard Bar and much more scaffolding was erected adjoining this area for other work.
Through a lapse of concentration this mature, seasoned industry worker climbed over the hard bar treating this as simply yet another obstacle to do his job in the area. Turning for some tools and stepping backwards within 2 footsteps of the floor opening and the certain fatal drop, a colleague shouted in time to witness him freeze, paralyzed with fear.
Hard Bar Barriers mixed with normal scaffolding can easily be mistaken. They must be taped.
It's amazing to see workers Not wear a face shield when using a grinder of cut-off machine. The temptation is that it may only be for a few seconds to do a job and yes they are already wearing glasses, so what's the problem? Face Shields are not only to protect from sparks!
Grinding wheels and cut-off wheels can and do EXPLODE. Faulty or damaged wheels spinning at thousands of revs per minute hit objects with horrific force. Your face is usually the closest. Don't let it be yours.
TIP: When starting or using a machine stand to one side or away from direct range.

In this industry it is dangerous to wear jewellery. Rings get caught, and rip the flesh from the finger bone. It's called "de-gloved" not a good sight. or feeling!
